Incident Report & Guidelines
The purpose of this incident report is to document and maintain any verbal or physical assaults as well as any other violations deemed inappropriate by the NCCSA, on members of NCSSA. This is to include tournament directors and his or her representative, managers, players, spectators and umpires.
The incident report form will be located on the NCSSA.CLUB website and can be downloaded for ease of completing the form.
The incident report will be completed by the tournament director and then forwarded to the chairman of the disciplinary committee for follow-up and/or possible action.
